Meridian works with Affinity Patient Coordination to create pharmacist-led care coordination to help members manage their medications. In addition, members who take six or more medications can enroll in a drug bundle program to have their medications delivered directly to their home.

To enroll in Meridian, call Illinois Customer Enrollment Services (ICES) at 877-912-8880. You can also register on the ICES website.

A primary care provider (PCP) will provide and manage your care. You’ll go to your PCP first for most health problems. Your PCP will work with you to get the care you need to reach your health goals.
Use our provider search tool to find a provider that’s available to you. You can also call Member Services. We are happy to help you find a PCP that fits your needs.
If your PCP thinks you need a specialist, your PCP will work with you to choose a provider. Your PCP will then arrange for your professional care.
